Anyone residing within the agreed practice boundaries may register with this practice.
To register, please collect a registration pack from the Practice reception, each person registering must have separate forms. For your convenience you may take the forms away and return them to us when completed.
Included in the pack is a short patient questionnaire, this allows us to provide medical care in the interim period, until your medical records are transferred from your old practice. You will need to make an appointment with a GP if you take regular medication.
If you are over 16 years of age, we also ask for you to bring proof of identity with you when you register. Acceptable documents include passport, driving licence or utility bill.
Choosing your doctor
Patients are registered with the practice, not an individual GP. Your medical card will be issued in the name of one of the doctors who will supervise your care.
You can at any time express a preference for a particular doctor, for all of your medical needs or on a case by case basis. This preference will be recorded in your medical notes and whilst we will always try to comply with your requests, it may not always be possible.
Accuracy of Medical Information
When we receive your medical records, they are summarised and the important information to assist with you medical care entered onto our clinical system. We realised of course that no-one knows your medical history as well as you do, and invite you to review the summarised data for accuracy. If you wish to see your summary please let a member of the team know and we will contact you when it is available.
