Information for New Patients Considering Registration with our Practice

The practice is currently accepting and welcoming new patients. That means that our list is open and we will be happy to register any patients who live within our catchment area and provide them with General Medical Services.   We have agreement from NHS Forth Valley and NHS Tayside to accept patients living within the following catchment area.

Catchment Area

The catchment area which the practice is contracted to provide General Medical Services to is below, and includes:

  • Blairingone
  • Carnbo
  • Coalsnaughton
  • Crook of Devon
  • Devonside
  • Dollar
  • Drum
  • Forestmill
  • Glendevon
  • Muckhart
  • Powmill
  • Rumbling Bridge
  • Tillicoultry

Unfortunately we are unable to register anyone living outwith this catchment area.   If you are currently a patient and move outwith our agreed catchment area you will be required to register with another practice.

Registration process

Registration with the practice is easy. Please collect the registration forms from the practice or download them below, and return them along with an appropriate form of identification.  As soon as this is done, you can then make appointments as required. If you have pre-existing medical or health conditions or are taking regular medication, please make a telephone appointment with one of our doctors or nurses to discuss how we can continue providing your care. 

Identification

We require two forms of identification when you register with the practice. 

  1. Personal ID — preferably photographic.  E.g. passports, driving licences or birth certificates. 
  2. Proof of Address

If you haven't got access to appropriate identification, please inform reception at the time of registration who will advise on what you should do. We will be able to register patients without identification in special circumstances.

Transferring your medical records

We will arrange for your medical records to be transferred from your previous practice after you have registered with us.   This usually takes around about 2 weeks if you're transferring from another Scottish practice.  If you're transferring from England or Northern Ireland, it can take longer. If you have come to this country from out-with the United Kingdom, we won't receive any medical records for you. Please send us any relevant information that you hold yourself.

If you would like to inspect the summary of your medical records, please make a request to our administration team.  No one knows you better than you do, so if there's something missing from your medical records or your summary, please let us know.

Once you're ready to register, you can download the forms from the Forms to Download page. There are different forms for each age group.

Once these are completed, please return them to the health centre with one of the forms of identification above.
