Changing Address

If you have changed address, please check that you are still living within the practice catchment area.  If you are no longer living within the area, you will need to register with another GP practice.

You will need to provide us with proof of address before we can update your details on our system.

Change of Contact Details

We contact patients primarily by phone, in order to provide test results, arrange review appointments, and to give other messages from the clinical and administration team.  It is very important that we have your current mobile and/or landline on our system.

Change of Patient Access Log-In Details

If you would like to change your login details with Patient Access for ordering prescriptions online, please log in to Patient Access.

If you have forgotten your User ID, or would like to register for the service, please phone us.

Updating Your Clinical Record

We would like to know about significant changes in your health and lifestyle.  If you are worried about changes in your health, or would like to discuss these with a doctor, nurse or other healthcare professional, please book an appointment.

If you would just like to add these changes to your clinical record, without reviewing or discussing them with a healthcare professional, you can use the form linked at the top of the page.  This will allow you to update your:

  • Height
  • Weight
  • Waist circumference
  • Blood Pressure
  • Resting pulse
  • Smoking habits
  • Alcohol intake
  • Whether you are a carer
  • Ethnicity
  • First language
